1. Print one copy to keep for yourself.
2. Print a second copy and provide it to the facility owner for filing.
3. For each facility you operate, print a copy to store and keep at each facility.
Item 3 above requires that a copy of your certificate be stored and filed at each facility your operate. This copy MUST BE READILY AVAILABLE upon request by an authorized UST inspector.
If a certificate is not printed at the end of the Certification Exam or it becomes damaged during the printing process or anytime afterward, or it is misplaced or lost, you must contact the Department of Commerce to request a replacement to be provided to you.
The Department of Commerce has the authority to rescind and revoke your certificate at any time.
